Two VISN 6 Medical Centers gain national recognition for patient safety

Durham - The VA National Center for Patient Safety (NCPS) located in Ann Arbor, Michigan has announced award winners for its 2008 Cornerstone Recognition program for completion of Individual Root Cause Analyses (RCA) and Aggregate Reviews.

The goal of the Cornerstone program is to enhance the RCA process and recognize leaders in VA patient safety.  Each VA Medical Center receives recognition according to applied levels of achievement in meeting the Cornerstone criteria. 

Staff at Beckley and Salem will be honored as a Bronze level recipient of the NCPS Cornerstone Recognition award for achievements during FY 2008.  Across VA, 71 facilities were recognized for FY 2008. There were four “gold” winners and 67 “bronze” recipients.  Bronze winners achieved the annual standard through timely submission of at least four single-case Root Cause Analyses (RCA) reports and four Aggregate Reviews focused on patient safety areas of concern.

Patient safety is a key goal of VISN 6 operations across its eight VA Medical Centers and 16 Community Based Outpatient Centers.
